Name, contact details and company information are optional. Provide only the information needed to understand and assess the concern.

Before you submit

A fair, respectful process.

Reports should be made in good faith and include enough factual information for the concern to be understood. Emplan will restrict access to the report, assess it impartially and avoid sharing identifying information more widely than necessary.

  • Describe what happened, when and where.
  • Identify the staff member or role if known.
  • Include witnesses or supporting records only where appropriate.
  • Avoid including sensitive personal information that is not relevant.

Selecting Submit opens your email application. Review the generated message, add any appropriate attachments, and send it to complete the report.

Confidential handling

The concern should be considered fairly, based on the available facts and without presuming an outcome.

If you provide contact details, they may be used only to clarify the report or communicate an appropriate response.
